Your Appointment Before your appointment, we ask that you download, fill out and bring all the necessary forms with you. Or, we can mail you the forms in a packet. Either way, it is always best to have all forms filled out before you arrive. Doing so will minimize your wait at the office and allow us to spend more time focusing on your needs. Arrive 20 minutes early to your appointment. It will give us a chance to ask you some additional questions and learn more about your needs. Privacy Policy: We encourage you to read and understand our Notice of Privacy Practices. If you have any questions, please bring them with you to your first appointment. New patient? Please bring the following: Medical Records: To help us evaluate your condition more precisely, please bring your medical records, any test results, x-rays, and other pertinent information. Personal Medical History: The more information you have, the better. You will be asked to fill out a patient history form when you arrive for the first time. New Patient Form: To complete the form, you will need information about your employer, insurance information, and contact numbers in case of emergency. Payment: Insurance co-payments are required at the time of your visit. If you do not have insurance, full payment is due at the time of your appointment or after treatment has been received. We also take Care Credit financing and have application forms and information available upon request. Patient Rights and Health Information Forms: These forms tell us whom to call with information, who can receive information about your health, and a brief statement/form about your patient rights. |
