Insurance

  1. •    Each Insurance Company has different policies and conditions regarding the procedures and conditions that are covered. We encourage you to contact your insurance company prior to your visit to discuss your policies coverage and requirements.

Please wear loose comfortable clothing and shoes.

Because Grants Pass Surgical Associates treats patients throughout Southern Oregon, it is at times necessary for your surgeon to respond to an emergency at the hospital. If you cannot wait because of this delay, we will gladly reschedule your appointment to the next soonest available time.

What to Bring to Your Appointment

    Please bring the following with you to assist us in providing you with a better consultation experience:

  1. •    Medical Records: Please bring your medical records, any test results, X-rays, and other pertinent information. If you are coming from out of town, please hand carry any X-rays or scans pertinent to your visit.

  4. •    Surgical referrals you received from your Physician

  5. •    Photo I.D.

  6. •    Co-Payment. Please feel free to discuss co-payments with our experienced staff at the time you make your appointment. We accept Cash, Check, Visa, MasterCard and Discover Credit Cards.
